Boston has one of the deepest behavioral health professional communities in the United States, anchored by the concentration of world-class academic medical centers — Massachusetts General Hospital, McLean Hospital, Boston Children’s Hospital, and the Harvard Medical School training programs that flow through them — and a dense ecosystem of universities whose counseling, social work, and psychology programs produce large annual cohorts of licensed clinicians. It is also a profoundly difficult place to staff a behavioral health practice. The cost of living in Greater Boston is among the highest in the country, the pull toward private practice is strong given the high commercial insurance penetration and the large affluent professional population that pays out-of-pocket, and the academic medical centers compete aggressively for the same licensed clinicians that community practices and group practices need.

Massachusetts licenses social workers as Licensed Independent Clinical Social Workers (LICSWs) — the highest level of social work licensure in the state, conferring full independent clinical practice. Massachusetts also licenses mental health counselors as Licensed Mental Health Counselors (LMHCs). The supervised hours requirements for both credentials are among the longer nationally.

McLean Hospital’s influence on the Boston behavioral health culture is significant. McLean — a Harvard Medical School affiliate and one of the most recognized psychiatric hospitals in the world — sets a clinical quality standard in the Boston market that shapes candidates’ expectations about the quality of supervision, clinical culture, and practice they should be able to access in employed positions. Organizations that can speak credibly to clinical quality in McLean’s terms attract candidates who might otherwise default to academic medical center employment.

Harvard, BU, BC, Simmons, and Northeastern produce enormous annual cohorts. The graduate social work and counseling programs at Boston University, Boston College, Simmons University, and Northeastern University collectively produce hundreds of MSW and counseling graduates annually who enter the Boston market. The competition among employers for the best graduates from these programs — through practicum relationships, alumni networks, and on-campus recruiting — is a strategic investment that pays dividends for years.

Massachusetts’ Medicaid (MassHealth) is one of the country’s more comprehensive programs. MassHealth provides relatively robust behavioral health benefits, creating a meaningful publicly funded sector that employs a significant share of Boston’s licensed clinicians. Community mental health centers and FQHCs competing with private practices for the same clinicians must differentiate aggressively on clinical culture, mission, and the specific populations they serve.
